Please complete this and send it to us after every shoot where you use your firearms.
Please note that shotgun information is NOT required.

Your full name

The date of the shoot (DD/MM/YY)

Range (eg: Sealand)

In the first column below, type the MAKE of your gun - "Ruger", "Parker-Hale", Lee Enfield, "not known" etc.

In the second column, enter the TYPE of gun - Rifle, Muzzle-loading pistol, etc

In the third column goes the serial number of that gun

In the fourth column, the type of use of the gun on that day :
Z = zeroing, P = practice, T = Testing,
and/or the name of any competition you entered eg: "Phoenix 100"
